Since NSAIDs and three biologics haven't worked, there are still some other options worth discussing with your rheumatologist: Other biologic classes that may still be options:
- IL-17 inhibitors like Cosentyx (secukinumab) or Taltz (ixekizumab), if not yet tried
- TNF inhibitors like Humira, Cimzia, Enbrel, Simponi, or Remicade — some of these also help with colitis, which could be a dual benefit worth exploring with your doctor

Non-medication approaches to discuss:
- Physical therapy and targeted exercise to manage stiffness and pain
- Corticosteroid injections for specific joints (short-term relief)
- Multidisciplinary pain management programs It's really important to advocate strongly for yourself with your rheumatologist, especially given how complex your situation is. A second opinion from another rheumatologist who specializes in spondylitis may also open new doors.
