It's completely understandable to have doubts when your MRI report uses cautious language like "likely" rather than giving a definitive answer. Let's break down what this means.

T2 hyperintensity in the subchondral region of the SI joints means there's increased fluid signal detected in the bone just beneath the joint surface. This is a sign of inflammation and is closely associated with sacroiliitis โ which is a hallmark finding in spondylitis. The reason your report says "likely" rather than confirmed may be because:
- MRI findings aren't always 100% specific โ similar changes can sometimes appear due to wear and tear
- "Active" sacroiliitis is a more specific term used when inflammation is clearly ongoing
- Spondylitis diagnosis is rarely based on imaging alone โ it combines symptoms, blood tests (like HLA-B27 and CRP), and clinical history
The good news is that MRI can detect sacroiliitis even before X-rays show any damage, which is actually a valuable early window for treatment. It's really worth discussing these doubts directly with your rheumatologist. Ask them to walk through your full clinical picture โ symptoms, blood results, and imaging together โ so you feel confident in whatever direction your diagnosis and care plan takes.
You deserve clarity, not confusion! ๐
