Members connected over the frustrating reality that many were diagnosed with axial spondyloarthritis or ankylosing spondylitis despite having mostly negative lab results, with only HLA-B27 positivity or imaging evidence of inflammation guiding their diagnoses. Several members shared that their rheumatologists relied on MRI findings showing bone marrow edema or sacroiliac joint inflammation, patterns of pain, and symptom history rather than elevated sed rate or CRP levels, which many noted stayed low even during active disease. A recurring theme was the struggle to be taken seriously by doctors who dismissed their pain as simple arthritis or misdiagnosed them with conditions like fibromyalgia, anxiety, or hormones, with many members emphasizing the importance of self-advocacy and finding providers who truly listen.

I do t have a diagnosis but the si pain is really getting me. Mri shows a lot of argthritis at si and lumbar. Just had my third nerve ablation July 2. Hoping it works. Not to optimistic now! I’ve only take NSAIDs and pain meds. Never took biological. I have taken prednisone in past and it helped a little while

My hla B 27 was negative however I was still diagnosed based on the level of pain and areas of pain I had been experiencing for years prior.

@A MySpondylitisTeam Member My rheumatologist said that not all people with spondylitis will have the high sed rate or high C-reactive protein. My sed rate was never measured and my CRP was low.

@A MySpondylitisTeam Member I was diagnosed with non-radiographic axial spondylitis mainly based on where I have pain and the HLA B27 positive test result. My pain has been in both SI joints and low back stiffness. I had an MRI of my pelvis and SI joints and that showed severe inflammation in both joints with bone marrow edema.
